I remember reading that he was sent to Purgatory, but does that count? Many people in the Marvel Universe have died and come back at one point or another, but has one the most violent and reckless heroes truely died and come back?
Wolverine
Character » Wolverine appears in 16088 issues.
A long-lived mutant with the rage of a beast and the soul of a Samurai, James "Logan" Howlett's once mysterious past is filled with blood, war, and betrayal. Possessing an accelerated healing factor, keenly enhanced senses, and bone claws in each hand (along with his skeleton) that are coated in adamantium; Wolverine is, without question, the ultimate weapon.
Has Wolverine ever really died?
" @damyria990: I think he died in the Days of Future Past story line. Other than that, Magneto almost killed him in Fatal Attractions. "
@Edamame: I remember reading both of those, too. I mean has he died in the 616 Universe?
" @Edamame said:No, at least not in the conventional meaning, turned into a vampire in a dracula storyline so he became "undead"." @damyria990: I think he died in the Days of Future Past story line. He's definitely died once during the Fall of Mutants saga, although he and his fellow X-Men were almost immediately resurrected by Roma for theri heroism. Maybe he was also dead for a moment in the 11th Uncanny X-Men Annual, but it's rather uncertain.
you can read some details following this link if you're not afraid of some nasty spoilers:

He also died during the original Secret Wars. But he and the other heroes were resurrected thanks to the healer chick and some of Battleworld's advanced tech.
One of the most ridiculous moments of such universe destroying-restorings was in Uncanny X-Men #107 (1977), when, due to the M'Kraan crystal, there was a consmic blink, making the whole universe nonexistent for a moment. I usually love Chris Claremont, but that was abit awkward if you ask me.
" He also died during the original Secret Wars. But he and the other heroes were resurrected thanks to the healer chick and some of Battleworld's advanced tech. "pretty sure Infinity Guantlet counts if along those lines as well.

In Chaos War He is Brain Dead
In X Men He is Vampire
In Wolverine Goes to Hell He is Possessed vy Demon
In X force He is Normal
And All this at The Same Time...
