Spawn attempts to let Major Foresburg out of his cage, when suddenly the disgarded creatures of the alley attack. A now weakened spawn is able to quickly put a end to these creatures, only now he has bigger problems to handle.
He has been pulled into Foresburg's mental war, constantly being waged within his own mind. Spawn attempts to save Foresburg, but the demons of his thoughts continue to chase them, without any sign of giving up. Foresburg asks Simmons for one last request.
