Is Red skull really red or dose he paint  his face ???

Normaly his head is just a skull that is read, but when he was killed by Winter Soldier, his mind was in Lukins body he wore a Red Skulls mask, right now his mind is in a robot body and has his Red Skull face on the robots screen that is on it's chest, but to answer the question, I would say no, he ether has a skull that is red or he wears a mask
