Is Red Skull's face a mask, or does his head look like that (and if it's his head, then how did it happened)?

I believe he's really that ugly... but I'm not knowledgeable about Cap or his enemies.

there have been tons of different Red Skulls so sometimes it's a mask and sometimes it's his face...it just depends on what body he's in at the time

Originally it was just a mask, but after he had his brain put into a clone of Captain America's body he got hit in the face with his dust of death and he actually looks that way
