Originally an angel who joined Lucifer's rebellion against God during his war in Heaven, Asmodeus was cast out like Lucifer and others, Asmodeus degenerated into a demon and came to rule a subsection of the nether realm Hell. He also mated with fellow outcast Lilith, parenting many demonic offspring, though most were slain by agents of Heaven.
Mephisto may have restored the true Asmodeus and consigned him to his lowly position. This may have been Asmodeus' first step towards regaining his power and his realm.

Who is the Third?
Although Lucifer Morningstar is always the Head of the Triumvirate and Beelzebub is always one of the other two lords, the third member isn't so defined. Originally, in The Sandman #4 where the three first appeared, it was Azazel. However, most likely an unofficial retcon took place in The Demon's second series for the purpose of fitting Etrigan's origin story better. So, ever since than, Etrigan's father, Belial has been the third and Azazel has seemingly ceased to exist in the comic book world.

The Dark Riders were a group of mutants, mainly Inhumans, who were Apocalypse's foot soldiers who strongly viewed his "survival of the fittest" ways.

Origin
Demetri was born in 1483. He is a Makai Demon that was banished to earth along with his castle Zeltzereich. The castle is only visible during full moon.
Powers
Demetri is a demon with vampiric abilities. He has none of the weaknesses vampires usually have, except that he is vulnerable to sunlight. That is why he constantly is seen with a force field. It appears as a thin glowing aura around his body.
He is able to turn humans into vampires, and they will be completely under his control. He is also able to turn males into females which can be seen during his Midnight Bliss move.
He normally appears as a human and seldom takes his demon form. He often assumes a bat-like form, and can assume the form of many bats at once.

The queen of hearts is a demonic entity that possesses and moves from person to person through a playing card. She serves under the god, Chaos, and wishes to be his queen.
Her name was once Jezebel. She resided in ancient Babylon where she served as the high priestess for the notorious Cult of Chaos. She charmed men and women alike with her enticing beauty. A trait she would use to lure them into her cult and then later sacrifice them to the Mad-god Chaos. Jezebel wound up doing most of Chaos bidding until she rebelled against him, demanding that she be his queen and most of all gain immortality. Angered by her demands, Chaos ripped the flesh from her body and trapped her very essence in a playing card as a punishment. Even though she got her wish of being immortal, she was bodiless and forced to be trapped in a "Queen of Heart" card for eternity. However, she does remain loyal to Chaos and still continues to serve him with human sacrifices preferably ten human hearts, which preferably must be evil.
