I think comics are simply meant to be entertaining. Sometimes that can mean just a fun story and other times something deeper. That can mean including elements of both fantasy and realism. Back in the golden age of comics there was more fantasy themes than anything else. It is still an important aspect of comics today, but times have changed and I think there is a desire for relevance.Â
  Concerning mass killings or civilian casualties in comics: I don't think it's necessary for every story, but there are instances where it makes sense and to leave it out would be unrealistic. When super-powered behemoths are battling it out in the middle of a city there are going to be some casualties. When a villain succeeds in his plans, there could likely be some deaths involved.
  Just because some writers "overkill" doesn't mean there isn't a time and place for mass deaths in comics. How else are you going to have a zombie apocalypse?
