According to the Guideline for Hazard Identification, Risk Assessment and Risk Control (HIRARC, 2008) hazard identification means the identification of undesired event that lead to the materialisation of the hazard and mechanism by which those undesired events could occur. Three method hazards identification process technique is as follows:
a) Observation
Observation is one method to identify the hazards that exist in the workplace. In observation, hazards can be identified by the behavior of employees, use of equipment, the employee doing the work, material / material used, and the work environment. During the observation, capture photos methods also used for obtain the evidence to indicate hazards exist in the workplace. Through evidence pictures, unsafe acts and unsafe conditions can be recorded as evidence to identify hazards. Besides taking pictures, making notes during observation also is an effective method to record what happens in the workplace. For example, taking notes and record all the work activities for up-slip vessel operation.
b) Feedback from Employees
Interviews and feedback forms are among the methods that can be used in hazard identification for this assignment. Interviews are carried out can be done by:
1. Direct interview with worker at site yard (Informal).
2. Formal interview with worker at office.
3. Formal interview with Head Department.
For feedback form, can be distributed by:
1. Given directly to the worker.
2. Given to the Head Department.
3. Provided at office counter.

c) Job Hazard Analysis
Job hazard analysis (JHA) break a job or task into specific steps, analyzes each step for specific hazard, develops safe procedures to eliminate or reduce those hazards, and integrates safe work procedures into safety and health programs. JHA must be developed for each job or task. Supervisor and worker must complete the JHA together. For example, do a JHA for Up-slip Operation at Cougar Shipyard and Engineering Sdn Bhd.
