Origin
Cain Marko's mom died when he was young and he was raised by his abusive father. With the (questionable) death of his father's colleague, Dr. Brian Xavier, Cain's father married Xavier's widow, Sharon. They moved into the Xavier Institute for Higher Learning in Westchester, New York.
Cain was a bully to Sharon's son, Professor X, due to the fact that his father clearly favored him over Cain. When Charles' mutant ability to read minds developed, he was able to feel Cain's jealousy of him. He was not pleased with the inadvertent invasion of privacy. During an argument with his father, Cain accidentally started a fire in his father's lab. The fire soon grew out of control. Cain's father, once again, showed his preference for Charles when he saved him first. After going back for Cain, he died of smoke inhalation.
Later, both Cain and Charles were drafted into the army and were in the same unit. On a mission in Asia, they were in a firefight and Cain deserted the unit. Charles went after him in the hopes of bringing him back before he got into trouble. Cain stumbled into a cave that had the lost Temple of Cyttorak inside. He discovered the Crimson Gem of Cyttorak and when he touched it, he was mystically transformed into Cyttorak's avatar, a living Juggernaut.
The mystical energies caused a cave-in and Charles and the world believed Cain to be dead. Cain eventually dug his way out and headed for the U.S. to get revenge against Charles. But not before killing the previous Juggernaut and slaughtering the village he was protecting to claim his full power. By this time, Charles had a team of mutants, the X-Men, that despite not being able to stop The Juggernaut manage to remove his helmet in the first encounter. Thus allowing Charles to subdue him.
After rebelling against Cyttorak, Cain began loosing the mystical energies that flowed through him for so many years. But since those energies were evil, they influenced much of his behavior as the Juggernaut. With that influence gone, Cain decided to turn over a new leaf and joined the X-Men.
While at the Xavier mansion, almost none of the X-Men wanted Cain there and some tried to force him out. But Cain stayed and would developed a strong friendship with a student named Sammy after Sammy saved Cain from drowning. Knowing Sammy's family issues, Cain saw himself when he looked at Sammy.
Juggernaut would apparently betray the X-Men by joining Black Tom Cassidy's new Brotherhood Of Evil Mutants, but he was only spying on them with the intent to warn the X-Men. But Sammy, not aware of what Juggernaut was doing, stumbled upon the group and was devastated that Juggernaut would betray everyone. Black Tom Cassidy killed Sammy, which caused Juggernaut to go into a murderous rampage towards his former friend. Eventually the 2nd Xorn would suck the Brotherhood into his black hole, in which Juggernaut willingly leapt into to protect Nocturne, another spy.
Juggernaut then joined the British super team, Excalibur, along with Nocturne with whom he developed a very strong bond with. Nocturne has displayed some romantic feelings for him, but he has yet to return them.
Cain has been suffering from depression due to his loss of power, and has recently retrieved the gem of Cyttorak. In doing so, Cain has also discovered that it was never Cyttorak's intention to use Cain as his avatar, but the true target was his brother Charles Xavier. How this event effects him and the team remain to be seen.
Cain was upset about the fact that it was meant for Charles and not him he fought with another possible Juggernaut before Excalibur came to take him home. Cain was meant to leave the gem behind but instead he took the gem back to England with him (heavily implying that he indeed killed his challenger) without the team knowing he took it.
Between World War Hulk and the last issue of New Excalibur Cain left without letting anyone know. In New Excalibur he told Brian he was leaving for personal reasons and Brian insisted that he take one of his planes, but before he left enjoy the party .
Powers
Juggernaut is powered by the Crimson Gem of Cyttorak, which contains a fraction of Cyttorak's power. With it he has immense strength and was even resistant to any physical harm, He can generate a force-field around his body, and virtually nothing except himself can stop him once he started moving.
His body can be harmed by strong-enough magic, but he heales instantaneously. He is also fully sustained by Cyttorak's mystical energies, making it so that he never requires food, water, oxygen, or rest.
The armor he wears is made of a mystical fabric. It is very durable and can be summoned at will. The helmet made from a metal from The Crimson Cosmos that shields Juggernaut from psionics, Juggernaut's only real weakness.
The full potential of Juggernaut's abilities are unknown even to himself, but he displayed various magical abilities that he didn't know how to replicate, such as: altering the size of matter, growing in size, tracking, levitation, absorbing energy, projecting energy, increasing his own strength, and creating portals.

In the Age of Apocalypse reality Juggernaut was a pacifist since he Xavier wasn't alive to project his anger towards and lived on Avalon. During a battle he was so conflicted about whether or not he should fight that he had a brain hemorrhage and died.

In the MC2 reality Juggernaut is married to Sachi Yama and have a son - J2.

In the Ultimate universe Juggernaut is a mutant and was a member of the Weapon X program and later the Brotherhood Of Evil Mutants. He's also in love with Rogue, another former Weapon-X member.

In the zombieverse, Jugernaut has been infected and fought the cosmic-powered zombies over Galactus' corpse. In this reality, Juggernaut's armor gave him his powers and anything his armor didn't cover was vulnerable, allowing zombie-Wolverine to "kill" him.
