After becoming accidentally bombarded with a massive dose of gamma radiation during the testing of an experimental bomb, Dr. Bruce Banner was transformed into the Incredible Hulk: a green behemoth who is the living personification of rage and pure physical strength.
He would die saving the universe by releasing his limitless gamma-powered energy against an epic villain; the hulk would die and leave Bruce with a radiation-induced cancer which would then kill him slowly (about 2 issues), painlessly and peacefully. (-_-,)

He should die saving the Earth......like, some gamma-powered satellite laser is going to waste a good portion of North America or something, but no one can reach it in time (since they're busy fighting said maker of satellite) so Hulk jumps up and destroys it, but in the process gets nailed with Gamma radiation that turns him back into Banner, and he falls to his death.
Rest of the superheroes go on to defeat super-menace.

I think he should have died at the apex of WWH. It could have been poignant statement about life in the Marvel Universe, and the subjectivity of what constitutes a hero. Unfortunately, I don't see Marvel ever killing him off, despite his extremely limited nature as a character. Plus, the death of Captain America kind of served that purpose already, even though he was a clear-cut hero.
