In the Exiles, I remember an Iron Man killed a Hulk by dropping a Gamma Bomb on him, but in retrospect, it only led to Wonder Man absorbing the radiation and becoming a stronger Hulk
That page where Batman kicks Hulk and knocks the wind out of him is just plain retarded. The abdominal muscles of a well built adult male are strong enough to withstand massive weight or trauma. Look at Harry Houdini. There is no way Batman could kick the Hulk hard enough to do that. Superman or Thor maybe, but not a mere human.

Then again, golden age Hulk was NOWHERE near as strong as modern age Hulk.
