A few examples children calling their parents by their name. Students calling their teachers by their name. Then people calling their bosses by Mr. something. I don't see how it's a sine of respect. Just because you call someone by dad or mom or Mr. teacher doesn't mean that person respects them. Respect is earned, not given.

It's because:
A. Using someone's name implies familiarity, so obviously if I'm not super close to my boss I won't call him by his/her name.
B. Hierarchy- The people you mentioned are higher up the food chain, therefore we just use titles.
C. "Mom" and "Dad" are just words in the language that describe what they do.

Ah thanks. You know, to my embarrassment, I always get confused with Chinese and Japanese culture. I have actually learned more about both cultures from the media than from real information lol

Curious point to make (and true). Yet the same isn't true, not really, when it comes to calling women, girls. Personally, I would prefer woman since I am an adult, but to be referred to as a girl isn't as insulting as it seems being called a boy is (if you're a man).
As for this thread, I think it's more to do with a line being drawn, to distinguish authority. More to do with school? I know that we referred to our teachers as "sir" and "miss", and if I call my parents by their first names, I would be talking funny for a week. Not condoning abuse, it's just how much of an insult my parents would take it.
For me, I prefer to be called by my first name but in my line of work, it would come off extremely unprofessional.
