Because it's an idealistic social concept defined and applied/attempted to be applied by people, hence it is limited by the subjectivity of those applying it as well as a natural amount of resistance from the people it's imposed on.
Apparently "create a new character" takes too much effort compared to "change the race/sex/whatever of an existing character".
