I know its cliche, but I say Human Centipede was the most messed up for me. Barely beating "Seed"
The most messed up movie you have seen?
Go read the Wikipedia entry on "A Serbian Film" and you know you won't take watching that movie.
I was about to bring that one up. Since I actually skimmed through the film after hearing about it, out of curiosity. It was on You Tube too of all places.
Tough to beat "A Serbian Film." I thought "Deadgirl" was pretty messed up as well.
And I'll jump on the cliche bandwagon and admit that I found several cringe-inducing moments in "The Human Centipede 2." The part where the baby was crushed under the gas pedal really got to me.
Hard to pick just one, so here are a few of the most messed up; Martyrs, Cannibal Holocaust, The Last House on the Left (Original), Irreversible, Ichi the Killer, and Salò.

Pretty sure I know the film you're talking about, I've seen some of it and it is f*cked on so many levels.

I haven't seen that many messed up movies. The Last King of Scotland was messed up, but that was intentional as they were trying to show you the horrors of the Idi Amin regime and truly scare you...And most of that was Whitaker's performance. AVPR was super messed up with the aliens eating the pregnant woman and kid, I really hate that disgusting movie.

Human Centipede and A Serbian Film are shock value garbage. The most messed up film (that's also quite good) that I've seen is probably Requiem for a Dream. A Clockwork Orange is another that comes to mind.
EDIT: Scratch that, I almost forgot. I watched a short film on Netflix awhile back that's borderline snuff. It's also pretty much shock value, but damn is it disturbing. It's called Aftermath. I'll describe it using only 3 words. Autopsy. Necrophilia. Cannibalism. Read the description on Wikipedia if you have to.
Human Centipede: Full Sequence. The first one was more funny than disturbing. Sure it was messed up, but it wasn't that bad, but Full Sequence was so rough I couldn't even sit through it

Sounds like a lot of people are really intimidated by torture porn. As for me the first thing that comes to my mind that I felt was really messed up, but it's not really correct to describe myself as feeling messed up, but more so as feeling completely and emotionally drained, and I don't mean crying your eyes out, but as if all sense of feeling has disappeared. That movie is Jacob's Ladder.
Human Centipede 2 is the only one I can think of shock wise in a movie. Requiem for a Dream showed what drugs can do to you and is great in terms of real life messed up kind of thing. Dark Days a documentary shows the homeless in an underground NYC sub station which is pretty messed up and it might still be on Netflix. Bestgore.com is a pretty real shocking website, but I WARN YOU NOW, IF YOU ARE EASILY DISGUSTED AND SCARED DON'T GO THERE!! It shows real life pics and vids of death and messed up stuff.

Depends on what you mean by messed up, Human Centipede if you mean just "what is wrong with this movie?" kind of messed up.
If you mean a film that actively unsettled me quite a bit, then I'd probably say either Last King of Scotland or A Clockwork Orange. Last King because it makes you like a character and then shows what a monster he is and Clockwork Orange because it makes you hate a character and then forces you to watch him suffer in the worst way.
1984 had some pretty unsettling stuff as well.
You have to have a special kind of self hatred to watch Visitor Q...... That movie is soooooooo messed up.
August Underground and Necromantic I & II spring to mind. Those early John Waters films were pretty sick too. Pink Flamigos and Desperate Living especially.

@judasnixon: Rightfully so. I remember watching GG Allin's Hated documentary with a friend of mine and him throwing up twice. It was hilarious. Really added to the over all viewing experience.

The full movie is on Youtube in it's horrid, uncut, uncensored, banned in multiple countries, vomit-inducing glory. I sat there glued to my computer screen for the whole thing thinking "oh please, how bad could it really be? You can only push the boundaries so far and get away with it. I'm a big boy, I can handle it." And I did, but GOD I wish I hadn't. Seriously, just the plot written on paper is cause for an uproarious controversy, but to see it all graphically depicted using (rather well done) cinematography that doesn't hold back at all is something no one should have to experience. There are those movies that are so shocking and so horrifying that you just have to watch because of the intrigue a movie like that creates. A movie you genuinely want to watch because you're almost daring yourself to sit through it. A Serbian Film is not, I repeat...IS NOT one of those movies. You think you want to see it, but trust me, you don't.
