The underwear outside looks better but I would prefer it inside for comfort.

To be fair, those outside underwear are actually called trunks, which from what I've heard, symbolizes strong men.

I think the better question is whether Superman now has fertility issues due to the wearing of such tight trunks for so many decades.

First of all, it's not underwear, people need to start understanding that.

However, I would most definitely wear the TRUNKS on the outside as it breaks up the costume nicely, if I were to wear a solid colored suit like Superman or Batman do.

Inside but I don't think Superman's suit requires that you wear underwear.

I'd wear armor so I wouldn't have to patch up my spandex after literally any fight I'd get in. Armor is more expensive and it would need occasional maintenance sure, but in the long run it makes more sense for someone with superhuman durability who is going to be getting hit by explosions and doesn't want to come out of them naked.
