Pretty much just as the title states, why does a super powerful telepath/telekinetic need guns? Just to look cool?

Yeah, he was birthed out of that ever so awesome time in comics when everyone had big guns, big swords and big hair.
The comic explanation is that because he uses his TK to keep his body together (what does he do when he sleeps?? makes no sense!) So he uses his military training and weaponry to get the job, whatever it may be, done.

But can't he still use telekinesis on a massive level even when he is holding his body together? And he can still mindrape anyone instead of using guns too.

The thing with Cable is the whole him being the child of Scott and Madelyne from the future wasn't conceived until after he had been around for several years. He was originally supposed to be just some crazy militant mercenary. So the whole retcon thing makes the usage of his abilities questionable. They probably tried to explain it as that he has to use all his concentration on his body. These are actually reasons why I am not too fond of the character.
There was a point in time when he was with the X-men that he used a weapon coupled with his psychic powers called a "Psymitar" or some crap like that.

Yes. And as a result, the guns stuck around. TBH, if we were to seriously consider most of these powers, guns and swords would be redundant but that wouldn't be as exciting for most readers of superhero comics.
