Is it about making someone feel the pain they've caused, or caused to innocents? Because I think that would be a big part of figuring out who is potentially immune to it. I've seen different definitions around. Also if the person feels guilt about something they wouldn't need to - let's say Superman killed a villain by accident but the dude was definitely not an innocent person - he'd feel guilt about it, but would it be something the stare would consider he actually should feel guilty about and make him feel the pain/suffering of (assuming of course suffering caused to innocents is part of it)?

I'm not really big on Ghost Rider so I'm curious about this.
