Manta killed his baby and sent his whole life into a downward spiral of crazy. So I'm gonna say manta.

Pre-New 52:
Black Manta killed Arthur Jr.
Aquaman's father died naturally.
New 52:
Black Manta killed Aquaman's father.
Arthur Jr. never existed, poor kid.
BM has affected AQ on a personal level more that any other villain. Either through the death of his son or his father (New 52).
But having said that BM is usually only relevant to AQ.
Orm on the other hand is AQ's brother and you can't get more personal than that. Furthermore Orm's actions always have far greater ramifications in the DCU than Manta's (he destroyed an entire city of Atlanteans, managed to rewrite reality, was the king of Atlantis (New 52) and forced AQ to abandon Mera to do something he hates (be the king) through his actions.
Everyone loses someone they care about at some point. Not anyone loses the love of his life, his way of life and is forced to do what he hates most, while sentencing his brother to jail.
Manta's actions might be more "hurtful" in a very specific way, but Orm's are more "globally important" so to say.
I'll go with Orm, but there's no doubt that AQ will most likely never hate another character as much as BM.
Kordax is, indirectly a big offender on sh*t that happened to Aquaman.

Infant son is murdered, wife goes crazy and blames him, wife tries to kill him, he loses his hand. Yeah, and people wonder why he was so gruff in the 90s.

He pretty much screwed Aquaman over by publicizing his abilities, as well as the origin and people of the lost city of Atlantis. Pretty much ruined his public image, especially after the events of Throne of Atlantis which leads the surface people to think that Arthur was behind the scheme. He's also considered one of the Atlantean's greatest threat (alongside Batman) and was a wanted man during the attack on the surface world. Stephen Shin not only ruined the people's overall perspective about him, but also leads the "surface-dwellers" to think if there is actually a underwater world (which comes evident during the THRONE OF ATLANTIS) and creates a bunch of exposure for Arthur which he doesn't need and want, especially after the events that took place.
Voted for Shin to be the minority on this poll, but Black Manta is such the obvious answer.
In my eyes, I think Ocean Master is considered to be the most greatest adversary of Aquaman's, and plays an important role as an antagonist for Arthur; but not someone that actually impacted his life the most.
@ckuakini: To be fair, people thought that Shin was nuts...which is why he ended up in the tabloids rather than broadsheet
@ckuakini: To soon for Shin. Very few stuff is known about him (his actual story when no obsessed with Atlantis). And so far he is not a villain. Just kinda d1ckish.
