For comics I will pick (Spoiler) Elektra turning out to be a skrull. The whole secret invasion is turning out to be awesome for me and this is the twist that started it.

The first good plot twist that popped into my head was the one at the end of Saw. Horror movies are always so predictable so this one i really didn't see coming.
