The above have all had long range or well concieved plans work out at one time or another, and usually usually prepare for many eventualities. They are hard to trap effectivly either because they have so many escape routes or have powers that they use in clever ways.
Note : A bad writer (Bendis, Reed and many others) can and have reduced even the greatest villains to the level of comic opera boob. This list can't be held accountable if someone decides to screw the villain for the sake of the plot.

I'd argue against some of these (Thanos? Prometheus?)...but I can't believe I forgot the Calculator! An old school villain who's finally gotten smart and started to play the game like he had a brain ;>)

He's just good with info and rubix cubes.
You ever see him invent anything amazing? Figure out who the Oracle is when she's sitting right in front of his face? He's not even in the top 5 smartest villains from DC's present, much less all villains from all companies.
I'm surprised so few people picked Kang and Tharok. Those guys are not only geniuses, they're considered geniuses hundreds of years into the future.

I don't know enough about Tharok, but I can tell you exactly why I considered Kang, then cast him on the rubbish heap. Kang is a great villain, but he's often been dipicted as just one of an infinite number of Kangs. They all flail and rage against the Avengers, in a way that seems comical and futile. Over and over again they attack - lose (or die) - and then start plotting again, with little effect. Doom is DOOM, and his actions aren't so difuse as Kangs. You can never be certain if your fighting the same Kang you fought before, but even if it's just a Doombot your battling...you can be sure that Doom sent it after you, and did so with a reason.
Kang was a better villain before they spread him so thin, but even if we compare him to Doom...Kang has few victories in his ledger, and Doom always seems to keep working towards a central plan. Doom challenged the Beyonder, and though that failed...it was with an audacity that inspires admiration. Doom rescued his mothers soul from Mephistos Hell. Doom struggles and plots eternaly to defeat Richards and the Fantastic Four. Has he done so? No...but he's come withing smelling distance of it...

I don't think Calculator was doing such a good job of monitoring Alex Luthor because he damn near destroyed the universe and Calculator didn't do a dang thing to stop it.

In no particular order. I cannot really include anyone like Joker/Doom/Luthor, because they do not have superhuman intellect.
1. Possessor (5 billion years old and has done nothing but gain knowledge)
2. Thanos
3. Brainiac.
4. The Leader (superhuman intelligence puts him above dooms human genius intellect)

Starman, who isn't a scientist of any sort and didn't even make As in elementrary school can correct equations dealing with superstring, dark matter and hyperspace because he learned about it in THIRD GRADE. Third graders 1000 years in the future are smarter than the "third smartest man" on the planet. Did I mention Starman is totally nuts and is still much smarter than Mr Terrific?
Granted, Mr Terrific isn't even close to the third smartest man on the planet even if you rule out time travelers, aliens, robots, etc. Still, nobody from the 20th century save T O Morrow and perhaps Sivana is any kind of match for super scientists from the 30th century, much less ones from the 64th.
5. Red Skull--he organized the attack that killed Captain America, and he organized Hydra, and he invented lethal poison gases.
4. Lex Luthor--He has no powers yet he finds a way to kick Superman's butt.
3. Doctor Doom--I mean, he's just a genius in every sense of the word.
2. Reed Richards--His inventions started the entire era of super heroes. Tell me thats not genius.
1. Kingpin--his criminal empire. The ability to be powerless yet still pound Punisher, Spiderman, and Daredevil. He has found a way to get the following people to help him: Daredevil, Bullseye, Elektra, Typhoid Mary, and infamously idependent Deadpool.

But my point is that even a moderately bright third grader from the 30th century (or is it 31st now?) is much much smarter than the "third smartest man" from our time.
TOMorrow can still make lists because he sees the future but just about anybody else from the present shouldn't be making these lists.
