Hulk doesn't have to throw Juggs.....there are different ways to send him going. While Juggs is pushing against him or throwing a punch, he just has to do a judo-esque throw to get him going. While their pushing against eachother, he just has to trip him or sidestep out of the way like in WWH/X-Men. The shield won't have much to do with it.

no, its not always there. You can't touch him if the shield is on.
As for walking into a lake, I think it is bad writing, Juggernaut should be in control of his own momentum, and should be able to stop at will. if he has gained the momentum to send him into a lake, he should be capable of exerting a similar kind of force backwards and stopping long before he reached it.
