Dorian's painting and the two golden medallions are not available for this battle.

Both are armed with their sword.

Fight takes place on the island Jack was deserted on.

Who wins between these two immortals?

Sparrow, determining by what we know of the curse of the Aztec gold compared to Dorian's immortality, Sparrow's limbs and other body parts are still useful and re-attachable where-as we assume Dorian's isnt.

I haven't read the novel in a while...

but didn't Dorian only want to have a way to maintain his good looks, and have all his bad goes into the painting or something?

and didn't he get scared when that guy was holding him up with the gun?

Jack Sparrow takes it, that is if we are talking about the Dorian Gray of the novel and not the film, though like others have said before, we do not know his limits in the film but I'd still say with confidence that Jack wins
