The slaughter of Chicagoans continues as Nighthawk's greatest foe enjoys his reign of terror. To catch the madman, Nighthawk must travel into the darkest bowels of the city...and into the darkest parts of himself!

While in a minimum-security hospital for a parole hearing, the formerly catatonic Steven Binst brutally assaults and murders Fabbo the clown then disguises himself as the clown to make his escape.

Five weeks later, Kyle Richmond, as Nighthawk, shakes down drug pushers in a Chicago flophouse in search of the source of a bad batch of crack cocaine that causes massive internal hemorrhaging in those who consume it. Everyone in the house dies, but Richmond finds a baby drowning in a blood filled toilet and saves its young life. He takes the child to Doctor Arredondo, who can barely manage a non-stop rush of dying junkies in the emergency room of Cook County Hospital.
