Batman is such an expert CQC fighter that he knows exacty where to strike to prevent death.

Batman knows what he's doing and how hard to hit, and where. And yes he should kill joker. Letting joker live guarantees dozens of more deaths.

Batman can cut off all your limbs and behead you, but he does it with just the right amount of precision and care that you don't die. Many thugs ambush him at once leading novices like yourself to believe he may accidentally kill one of them. But fear not sir Batman has 200% control over each cell in his entire body. The more enemies, the less likely he'll kill one, which he never does. He did train and master in at least 300 fighting styles you know. You underestimate how hard writers are willing to try to worm their way around that and how inexplicably willing people are to go along with their circuitous and tortured logic in the name of keeping the official kill count at zero. But yeah, there is literally no chance that at least a few of them have not died. Even if Batman didn't kill them on the spot they still died due to complications from their wounds. Infections, internal bleeding, organ failure, etc. There is no such thing as a harmless knock-out. That's how people get brain damage. But the power of the willing suspension of disbelief says that Batman is so precise that he can consistently beat legions of criminals within an inch of their lives over and over without a single direct or indirect casualty.

Its also surprising that he newer killed some one with explosives or a Batarang. In the new 52 they even have a villain named Bat Head in Batman and Robin, who has a Batarang in his head.

Yes, the odds are very good, extremely in fact. And let's be honest with ourselves, those of us that have played the Arkham games know that that version of Batman is really just going around on one big killing spree.
